SW 580-209: LOCATION: via Internet. Full title of course: "Inter- professional Collaboration in Transitions of Care." This course will provide social work students with an online asynchronous inter- professional practice experiential forum within which to explore the characteristics and implications of collaborative inter-professional practice in helping clients through transitions of care. Eligible students will be recommended by COSW faculty/program directors and will participate in an online module based IPE group process to build knowledge and skills related to working with client transitions for health, mental health and health related issues. Students will be required to attend two face-to-face sessions: one to introduce them to their inter-professional team and one to present their final transition of care plan for their identified case study.<\/p>
